What is the timeframe for submitting the request form? I can't find it in writing, but a minimum of 1 week out is sticking in my brain.

NCHSAA did not put a time frame to submit the form in to them in the Handbook.

Teams may allow varsity athletes from the same team to participate in two different tournaments on a given day.
(i) May not exceed other established weekly limitations.
(ii) Tournaments must be conducted on the same day.
(iii) Applies to Varsity only
(iv) Teams may use this provision a maximum of three (3) times per season.
(v) Teams must submit a request to the NCHSAA, which will be posted to the NCHSAA website
(vi) Teams may not participate in two dual team tournaments on the same day
